Registration

Helmsley Medical Centre welcomes new patients living in or moving into our Practice area. You can register by either calling into the Surgery and completing a registration form or by printing your own form from the Downloads section of our website. You must include full details of name, date of birth, NHS number and address, along with information about your previous Doctor. You will be required to complete a form for each family member registering at the Practice.

As part of our contractual requirement Practices in England are required to allocate all registered Patients a named GP who has overall responsiblity for coordinating the services that we are able to provide to you. This will not affect your care in any way or prevent you from seeing any of the Medical Professionals available at the practice. If you wish to be informed of your named GP please ask at reception.

The Practice has an electronic link to the registration department of the Family Health Service via NHS England who oversees the transfer of Patient records from one GP to another (GP2GP). Your medical records will be requested from your previous GP and will then be sent to us electronically a lot sooner. You can however access medical services from the time of registration, you do not have to wait until the Practice has received your paper medical records from your previous GP.

Temporary Registration

If you are ill while away from home or if you are not registered with a doctor but need to see one you can receive emergency treatment from the local GP practice for 14 days. After 14 days you will need to register as a temporary or permanent patient.

You can be registered as a temporary patient for up to three months. This will allow you to be on the local practice list and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with that practice.

To register as a temporary patient simply contact the local practice you wish to use. Practices do not have to accept you as a temporary patient although they do have an obligation to offer emergency treatment. You cannot register as a temporary patient at a practice in the town or area where you are already registered.

It would be helpful if you print off one of the forms, complete it and  bring it with you to the surgery if you wish to be seen.  If this is not possible you can complete a form at the surgery.
