If your doctor has referred you to a specialist for further care you will be asked to book your appointment using the 'NHS e-Referral' system.
You will receive notification from us within 2 weeks when your e-Referral paperwork is ready for collection, which gives details of how to book your appointment and any passwords you may need.
We can email these details if you have provided the practice with an email address.
To track your e-Referral you will need the following details to log into the e-Referral website:
- Booking reference number (For example, '000286522495')
- Year of birth (For example, '1965')
- Access code or Password (This is case sensitive and may require a space. For example, 'Word pass')

Have you got a non-urgent problem with your feet?
You don't need to come and see your GP anymore! Our patients can refer themselves directly to Podiatry at County Durham and Darlington Foundation Trust to be seen and treated.
All you need to do is complete the below link or email [email protected] they will then send you an electronic referral form to complete and return to them. You can also contact them by telephone on 01388 455 200
This could save you up to 3 weeks' worth of waiting by avoiding the need for a GP referral.
